Authorizing an Agreement with The Children's Shelter
 
 
SUMMARY:
 
This Ordinance authorizes an agreement with The Children's Shelter to provide case management services for the San Antonio Metropolitan Heath District's (Metro Health's) teen pregnancy prevention initiative, the Teen HOPES Project, for a total amount not to exceed $1,005,900.00.  The case management services will be provided to teen mothers through the evidence-based Healthy Outcomes through Perinatal Education and Support (HOPES) model in an effort to reduce repeat teen pregnancies in San Antonio, Texas. The term of the proposed agreement begins January 30, 2014 and ends on September 30, 2016, and includes an option to renew for one additional year.
 
BACKGROUND INFORMATION:
 
On January 31, 2013, City Council authorized the submission of six project proposals by Metro Health through our Regional Healthcare Partnership Anchor entity, University Health System (UHS) for consideration and funding in the amount of $43,392,942.00 through the new Delivery System Reform Incentive Payment (DSRIP) arm of the "Texas Healthcare Transformation and Quality Improvement Program" or "Transformation Waiver". Metro Health carefully selected projects in the following areas: diabetes prevention, children's oral health services, HIV and Syphilis prevention, breastfeeding promotion, teen pregnancy prevention and neighborhood health promotion. All six project proposals are in alignment with Metro Health's strategic plan and many community goals set forth by SA2020 and have the overall goal of improving health outcomes.
 
In 2010, San Antonio's Bexar County birth rate for females ages 10-14 was 0.7/1,000 and was 75% higher than the national rate 0.4/1,000. According to the latest comparable data, Bexar County's 2008 pregnancy rate for females ages 10-14 was 1.9/1,000 and was 36% higher than the national pregnancy rate of 1.4/1,000. The number of Syphilis and HIV cases among youth ages 13-19 increased by 78% in Bexar County from 2006 to 2010. During the same period, reported cases of Chlamydia and Gonorrhea increased by 31% for the same age group. In 2010, seven zip codes had teen birth rates for females ages 15-19, 3 to 4 times the national teen birth rate of 76.9/1,000 females age 15-19. Metro Health's 1115 Waiver projects include activities for the reduction of teen pregnancy in San Antonio which is consistent with the SA2020 community vision, the Bexar County Community Health Improvement Plan, and the Metro Health strategic plan.
 
In October 2013, Metro Health in coordination with the Finance Department's Purchasing Division issued a Request for Proposals (RFP) for case management services for the Teen HOPES Project.  One qualified proposal was received and evaluated by the evaluation committee comprised of representatives from Metro Health.  The Children's Shelter was selected by the RFP evaluation committee as the recommended Respondent for award.  The evaluation committee based its decision on several factors, primarily those of Experience, Background, and Qualifications; Proposed Plan; and the SBE Subcontracting Program.    
 
Under the proposed agreement, The Children's Shelter will utilize the HOPES model with the Parents as Teachers (PAT) home visitation curriculum to enroll and serve a minimum of 100 teen parents per annual funding cycle.  The ultimate goal of the project is to reduce repeat teen pregnancy among teen parents. Eight domains are identified in the Teen HOPES Project as contributing to risk for future pregnancies, with the domains of Reproductive Life Planning and Life Course as a primary focus. The PAT educational curriculum encourages healthy parent/child relationships and decreases risks for abuse and neglect.
This contract will be awarded in compliance with the Small Business Economic Development Advocacy (SBEDA) Program, which requires contracts be reviewed by a Goal Setting Committee to establish a requirement and/or incentive unique to the particular contract in an effort to maximize the amount of small, minority, and women-owned business participation on the contract. The Goal Setting Committee applied the Small and Minority/ Women-Owned Business Enterprise Prime Contract Program with twenty (20) evaluation preference points. The Children's Shelter did not receive any points since they are not certified as a SBE and/or M/WBE. Additionally, the Goal Setting Committee also set a ten percent (10%) percent Small Business Enterprise (SBE) subcontracting goal. The Children's Shelter has committed to 10% SBE subcontractor participation.
 
 
ISSUE:
 
Metro Health is requesting City Council authorization of an agreement with The Children's Shelter to provide case management services for Metro Health's teen pregnancy prevention initiative, the Teen HOPES Project, in order to satisfy a deliverable of the Medicaid 1115 Waiver which provides funding for this agreement.
 
Authorization of this agreement will allow Metro Health to move forward with the implementation of goals set forth in the Metro Health strategic plan and Medicaid 1115 Waiver project.
 
 
ALTERNATIVES:
 
Should the contract with The Children's Shelter not be approved, Metro Health will be unable to move forward with its plan for January 2014 implementation of teen pregnancy prevention case management services and may be unable to meet deliverables set forth in the Medicaid 1115 Waiver project.
 
 
FISCAL IMPACT:
 
Metro Health is requesting City Council authorization of an agreement with The Children's Shelter to provide case management services for the San Antonio Metropolitan Heath District's teen pregnancy prevention initiative, the Teen HOPES Project, for a total amount not to exceed $1,005,900.00 for a term, beginning January 30, 2014 and ending on September 30, 2016.  
 
Funds will be appropriated annually, by fiscal year, and expenditures for case management services are estimated to cost $335,300.00 annually for a total not to exceed the amount of $1,005,900.00.  Funding for this agreement is available and budgeted within Metro Health's Medicaid 1115 Waiver fund and there will be no impact to the General Fund.
 
 
RECOMMENDATION:
 
Staff recommends approval of an ordinance authorizing an agreement with The Children's Shelter to provide case management services for the San Antonio Metropolitan Heath District's teen pregnancy prevention initiative, the Teen HOPES Project, for a total amount not to exceed $1,005,900.00 for a term, beginning January 30, 2014 and ending on September 30, 2016, and includes an option to renew for one additional year.
