Authorizing an Amendment to the Family Service Association Agreement
 
 
SUMMARY:
 
This Ordinance will authorize an amendment to the Family Service Association (Family Service) agreement to increase the level of compensation provided by Family Service to the San Antonio Metropolitan Health District (Metro Health) by $21,965.00 to a cumulative amount not to exceed $96,860.00 for a term that began on October 1, 2013 and ends January 31, 2015.  The proposed amendment will expand the scope of services provided by Metro Health to include (1) data entry for all dental evaluations and records related to services provided and (2) case management for children that require immediate care.
 
BACKGROUND INFORMATION:
 
On an annual basis, the Metro Health Oral Health Program provides diagnostic and preventive care to more than 12,000 children living in poverty by providing services in non-traditional settings, such as Head Start and on campuses of local Title I elementary schools.  Head Start is a federally-funded program whereby Family Service provides day care, education, child development, nutrition and social services, health and disability assessment and parent involvement, both on a full and part-time basis.  These programs are designed to serve children ages 3 to 5 years whose parents' income meets Administration for Child, Youth and Families Guidelines.
 
Approximately 23% of children enrolled in the City's Head Start Program are not covered through Texas Medicaid or the Children's Health Insurance Plan (CHIP). During the 2010-2011 school year more than 1,850 children that are enrolled in the program were identified with untreated decay. Case management services are provided by Metro Health dental staff to assist participating children to receive needed treatment and to connect families to a source of ongoing care in the community.
 
The Metro Health has provided Head Start dental services for Family Service for the past several years.  City Council most recently authorized a dental service agreement with Family Service on September 19, 2013 for Metro Health's provision of oral health services, including oral health assessments and preventative care such as fluoride varnish applications, for children enrolled in the City's Head Start Program.
 
 
ISSUE:
 
The parties wish to amend the current contract to increase the level of compensation provided by Family Service to Metro Health in order to expand Metro Health's scope of services to include (1) data entry for all dental evaluations and records related to services provided and (2) case management for children that require immediate care.  This amendment will help ensure that those children who need additional or specialized care receive it in a timely manner.
 
 
ALTERNATIVES:
 
If this contract amendment is not authorized, Metro Health will need to identify alternative mechanisms to ensure children that require specialized or urgent care receive it in a timely manner.  In addition, Metro Health will forgo $21,965.00 in additional compensation to provide dental services for Head Start enrollees.
 
 
FISCAL IMPACT:
 
Metro Health will receive an additional $21,965.00 from Family Service for the provision of dental services through the proposed amendment.  The cumulative amended amount will not exceed $96,860.00 for a term that began on October 1, 2013 and ends January 31, 2015.  The City will bill Family Service on a monthly basis for expenses incurred.  
 
The current agreement, which was previously approved by City Council on September 19, 2013, requires in-kind services in an amount up to $16,665.00 for the period beginning February 1, 2014 and ending January 31, 2015.  The proposed amendment will not affect the in-kind services requirement, which is funded through the General Fund.
 
 
RECOMMENDATION:
 
Staff recommends approval of a contract amendment to increase the level of compensation provided by Family Service Association to Metro Health for cumulative amount up to $96,860.00 for the provision of oral health services to Head Start enrollees for a term that commenced October 1, 2013 and terminates January 31, 2015.
